Social Workers are licensed professionals who provide case management services to children, families, the elderly, and other individuals within local communities and institutions.
Incumbents possess a degree of knowledge and proficiency sufficient to perform work independently with little or no additional training.
The Nevada Department of Veterans Services (NDVS) is recruiting for a Social Worker 2 position geographically located at the Southern Nevada State Veterans Home in Boulder City, Nevada. The Social Worker 2 is responsible for providing case management services to residents of the Nevada State Veterans Home. The incumbent will assess residents upon admission to provide optimal functioning and maximum adjustment with the facility, develop individual care plans for residents, address Minimum Data Set requirements and complete summaries for status changes, and readmissions; complete progress notes on a quarterly or as needed basis addressing concerns, assist residents in adjustment issues of a psychosocial nature, and establishing a discharge plan for residents. Experience and/or knowledge of MDS 3.0 in a long-term care facility and knowledge of electronic charting is preferred.
