(Reno, NV) – The Sanford Center for Aging at the University of Reno, Nevada is now offering comprehensive geriatric assessments to those age 65 or older. Appointments for these assessments are now open to the public with a physician referral.
The clinical team consists of a geriatrician, social worker and pharmacist. They will provide what is known as a “whole person” assessment including the following:
Personal health history and review
Physical assessment, including risk for falls/frailty, cognition and depression
Behavioral/psycho-social assessment including memory issues and dementia
Medication therapy management review
A personalized care plan to share with other care providers
Referrals to other providers or services, if needed
A chronic care management program, for those who qualify
The clinic asks referrals fall under two or more of the categories/conditions below; however, a spokesman for the program said clinicians will look at each individual request. Here are those categories/conditions:
Age 65 or older
Living alone
Unexpected weight loss
Increasing weakness
Decreasing mobility
Memory complaints
Multiple chronic conditions
Multiple chronic medications
Frequent hospitalizations
To receive an assessment, you must have a physician referral. You must also understand this assessment will not replace your primary care physician.
